CIM Forms

*** Important applicant notice:

Applicants for the 09-10 year, please note that the 09-10 forms are not yet available. They will be posted on or around January 1, 2009. Please do NOT submit prior year forms as they will not be applicable. Forms are year specific. We cannot use 08-09 forms for the 09-10 year.

All 09-10 applicants will get a student-specific e-mail from the CIM Financial Aid Office mid-December with more detailed information about the forms required and their due dates. Generally, most 09-10 aid forms will be due on February 15, 2009, but will not be available until January 1, 2009, as you will need 2008 year end data to complete them.

FAFSA - online at www.fafsa.ed.gov Required each year to apply for or to renew aid for U.S. citizen and eligible non-citizen applicants/students.

09-10 applicants please note the 09-10 FAFSA will not be available until 1/1/09.

08-09 International Student Declaration and Verification Form This form is required of all international students applying for admission for the 08-09 academic year.

The 09-10 Declaration and Verification Form will not be available until 1/1/09. 09-10 applicants should NOT submit the 08-09 form.

US Enrollment Verification Form Required of all international applicants who are currently or have attended another U.S. school (high school or college).

Transfer Clearance Form is required of any applicant who will NOT have completed their current degree at their prior school before enrolling at CIM. This is also required if you have previously deposited and signed an Intent to Enroll Form with another school (this may happen in you are wait-listed at CIM, and accepted another school’s offer in the meantime).

2009-2010 Intent to Re- Enroll and Renewal Aid Form. Required of every returning student by February 1, 2009.

2008-2009 Work Study Contract - required of all Work Study awardees - each year. Only complete if asked to do so by the CIM Financial Aid Office.

Special Circumstances Appeal Form Use this form to appeal for a re-evaluation of your financial aid package. You must have specific, documented financial changes either to student or parent finances or family situation that is new in 2006 from the data submitted on your current FAFSA.

Outside Scholarship Information Form - use separate form for each outside award – outside awards are always credited one half to each semester, like all other financial aid, irregardless of the disbursement frequency from the source of the award.

Private/Alternative Loan Rate and Terms Comparison (PDF) Although you may use any lender of your choice, this document compares information from several lenders often used by CIM students.

FERPA Release Form - To permit CIM to speak to a parent, spouse, other person about student-specific information (such as tuition balance due, financial aid, grades, etc.) CIM must have the student consent in writing. FERPA regulations only apply to confirmed students (law does not apply to applicants, until they have confirmed enrollment). All admitted students are sent this form, but need only return to CIM if/when confirming intent to enroll.
